It’s an even bigger problem for African-Americans, whose prevalence of HBP is among the highest in the world. Unfortunately, only 54.1% of the people being treated for HBP have it under control. In an effort to help Americans manage their blood pressure, the American Heart Association/American Stroke Association is launching Check. Change. Control. Connecting Communities and Care with support from and in partnership with Kaiser Permanente . Our goal is to promote HBP management by partnering with clinics and community-based organizations that serve African-Americans and other populations at risk for HBP. The AHA and participating organizations will encourage patients to track blood pressure readings, share them with their caregivers, volunteer health mentors and physicians, and monitor their progress.
